Customer Responsibilities and Checklist

When submitting notes to PMG, please follow these instructions:

  • Be sure to print your name and your PMG member account number (i.e. CS123456) in the space provided on the submission form.
  • Provide contact info (phone number, postal and email address).
  • Select type of submission. Choose only one: US or World.
  • Do not mix raw and holdered notes on one submission form.
  • Select one grading tier.
  • Submit each grading tier on a separate submission form.
  • Submit the following on separate submission forms: Fancy Serial Numbers (e.g. 88888888, 123123, 12344321), Errors, Oversize Holders and Custom Holders.
  • Notes must be listed on the submission form with the proper catalog number.
  • The Friedberg or Pick number should be referenced on the submission form when submitting notes in either of these references; if they are not, a fee of $5 per catalog number will be charged.
  • For regular submissions, list each note on a separate line. For bulk submissions, list each catalog number on a separate line. Do not list more than 50 regular note submissions or 100 bulk note submissions per form. If you are submitting notes under the same tier and run out of lines, use additional submission forms as necessary and staple them together.
  • For mixed bulk submissions of 50 or more notes, put the total in the "Qty." column and attach an Excel spreadsheet to the form with the details of all notes in the submission.
  • Include declared value of each line item and total at the bottom of the form.
  • If you are requesting a Pedigree, print clearly and legibly on the corresponding line in Box 9. For example, if you want all caps, print the pedigree in all caps.
  • If you have a PMG member credit, it will be automatically applied to your submission fees. You must still provide a valid payment method to be used for any amount due above the member credit.
  • Place only one note in a sleeve for regular submissions or ten per sleeve for bulk submissions.
  • Unless requested otherwise by the customer on the submission form, PMG will generally encapsulate notes in the smallest possible holder if doing so allows them to fit in one of PMG’s standard holder sizes. For example, a vertically oriented note will be encapsulated in a horizontally oriented PMG standard holder if it will fit. Notes that require the PMG Oversize Holder or Custom Holder will generally be encapsulated according to the note’s orientation.
  • Place raw notes into non-PVC plastic sleeves. Use larger sleeves for larger notes. Notes should insert easily into sleeves. Placing a note in a sleeve that is too small may result in damage to the note. Remove all other packaging before placing a note in a sleeve. Do not cut, tape, staple or glue sleeves as that may damage a note.
  • If submitting holdered notes that were graded by another company and need to be removed from their holder(s), complete and include the Consent To Remove Note(s) From Holder(s) Form.
  • Package your notes in the order that they are listed on the submission form.
  • Sign and return all required forms with your submission.
  • IMPORTANT: If you would like to pick up your submission at a FedEx location, you must enter the FedEx location’s address (along with your name). For security reasons, PMG will not change the shipping address or instructions after the submission has shipped from our facility. Any undelivered shipments returned to PMG will be subject to additional shipping fees charged to the submitter.
  • INTERNATIONAL SHIPMENTS: You must designate yourself as the “importer of record” for any shipments to PMG, and you are responsible for all customs duties and fees for your submissions. If PMG incurs any customs duties and fees for your submissions, these costs will be billed to you plus an administrative fee of $25.
  • Turnaround times are approximate and are not guaranteed.
  • IMPORTANT: If paying by check, make check payable to PMG and indicate check number in the appropriate area. If paying by wire transfer, indicate that in the appropriate area. If paying by credit card, include credit card information and sign on the appropriate line.
  • Note: Boxes 11 & 12 must be signed in order to process your order. Incomplete forms will result in a delay in processing.
